Job Purpose: To support adults using Intermediate Care services to regain independence, confidence and daily living skills, helping them live safely in their own homes for as long as possible.  

Duties  
- Support service users in their own homes with personal care, meals, mobility and daily living tasks.
- Encourage independence by supporting people to do as much as they can for themselves rather than doing tasks for them.
- Work with short-term, intensive care plans focused on recovery and rehabilitation.
- Review progress regularly and report any changes in health or wellbeing to senior staff.
- Keep clear and accurate paper and electronic records in line with policies and legislation.
- Support safe storage and administration of medication in line with training and procedures.
- Work closely with families, carers and other health and care professionals.
- Attend care reviews, discharge meetings and planning sessions when required.
- Work flexibly across the Isle of Man based on service needs.
- Work on a rota including evenings, weekends and bank holidays.
- Respond appropriately and calmly in emergency situations.

Essential Skills
- Good spoken and written English communication skills.
- Ability to work independently and as part of a team.
- Ability to manage emergencies and changing situations.
- Basic IT skills, including using electronic record systems.
- Commitment to dignity, respect, choice and person-centred care.
- Willingness to complete mandatory training and work towards QCF Level 3 if not already achieved.
- Full driving licence with access to your own vehicle.
- Physically able to meet the demands of the role.
- Satisfactory police check.

Desirable Skills
- NVQ or QCF Level 2 or Level 3 in Health and Social Care.
- Experience supporting older people or adults with complex needs such as dementia, mobility issues or stroke.
- Experience working in health, care or community-based roles.
- First Aid, Moving and Handling or Dementia Awareness training.
- Experience working as part of a multi-disciplinary team.
