Job Purpose: Support people with complex needs to regain and maintain as much independence as possible in their own homes. You will provide short-term, person-centred care across the Island, helping people with everyday living and supporting their rehabilitation goals.  

Duties  
- Support service users in their own homes, including lone working in the community.
- Help people work towards agreed goals so they can live as independently as possible.
- Encourage service users and their families to stay involved with their Intermediate Care programme.
- Support people returning home following a hospital admission or respite care.
- Provide personal care, including hygiene and continence support.
- Help with everyday tasks including food preparation, cleaning, shopping, collecting pensions and paying bills.
- Support meal planning, preparation and individual dietary requirements.
- Support medication management in line with procedures, including arranging repeat prescriptions when required.
- Monitor service users' progress and report changes in their health, needs or circumstances.
- Work alongside healthcare professionals and other services to support rehabilitation and independent living.
- Attend discharge meetings and service user reviews when required.
- Act as a key worker and contribute to person-centred support plans.
- Encourage service users to make informed choices and take an active role in their own care.
- Carry out basic risk assessments and support positive risk-taking.
- Maintain accurate paper and electronic care records.
- Communicate with families, carers and other professionals.
- Respond appropriately to emergency situations.
- Attend team meetings, supervision, appraisals and required training.
- Work flexibly across the Island, including evenings, weekends and Bank Holidays.
- Follow safeguarding, health and safety and care policies and procedures.

Essential Skills  
- Commitment to achieving QCF Level 3 in Health and Social Care if not already attained.
- Good written and spoken English communication skills.
- Good IT skills, including Word, Outlook and Android.
- Ability to work independently and deal with emergencies.
- Ability to work effectively as part of a team.
- Understanding of positive risk-taking.
- Reliable and trustworthy approach.
- Positive and friendly manner.
- Commitment to training and professional development.
- Highly motivated and person-centred.
- Understanding and appreciation of individual needs.
- Responsible and professional approach to work and colleagues.
- Full, valid driving licence with access to your own vehicle.
- Ability to work varied hours.
- Willingness to work anywhere across the Island according to the needs of the service.
- Physically able to carry out the duties of the role.
- Satisfactory police check.

Desirable Skills  
- NVQ/QCF Level 2 or Level 3 in Health and Social Care.
- Basic First Aid.
- Safer People Moving and Handling training.
- Dementia Awareness training.
- Experience working as part of a multidisciplinary team.
- Understanding of rehabilitation.
- Experience supporting older people with complex needs such as dementia, mobility difficulties or stroke.
- Positive approach to change.
- Awareness of dignity, choice and respect.
- Ability to communicate effectively with families and other professionals.
- Isle of Man Worker status.

Salary & benefits

£29,336 - £35,386 per annum (pro rata for part time) or £15.19 - £18.32 per hour plus an additional 8.33% rolled up holiday pay (casual)

Full Time - 37 Hours; Part Time - 14 Hours; Casual / Relief - Zero Hours
